Transcript Request
An official transcript is a signed and sealed document which goes directly from the Faith Academy Online Guidance Office to a third party such as a school or employer. It must be mailed by us directly to that third party, OR if you wish to deliver it yourself, must remain in the sealed envelope in which you receive it.
To request your high school official transcript
Please click on the link below to fill out an online web form and pay with a credit card. Please fill out the form completely so that we can fulfill your request most efficiently. Allow at least 5 school days for processing of transcript requests. Transcript requests are not processed during school breaks. There is no immediate or rush service available. The fee for each transcript request is $5. The payment options for the web form are Master card, Visa, Discover and American Express.
